چکیده

WARNING: DIMINISHED ANTIPLATELET EFFECT IN PATIENTS WITH TWO LOSS-OF-FUNCTION ALLELES OF THE CYP2C19 GENE See full prescribing information for complete boxed warning. • Effectiveness of Plavix depends on conversion to an active metabolite by the cytochrome P450 (CYP) system, principally CYP2C19. (5.1, 12.3) • Tests are available to identify patients who are CYP2C19 poor metabolizers. (12.5) • Consider use of another platelet P2Y12 inhibitor in patients identified as CYP2C19 poor metabolizers. (5.1)

Should Genetic Testing Be Done in All Patients Treated With Clopidogrel and Undergoing Percutaneous Coronary Intervention? CYP2C19 Genetic Testing Should Not Be Done in All Patients Treated With Clopidogrel Who Are Undergoing Percutaneous Coronary Intervention

Clopidogrel when added to aspirin reduces major vascular events in patients undergoing percutaneous coronary intervention (PCI).1 Recent reports have suggested that common genetic variants involving hepatic cytochrome P450 system enzymes that convert clopidogrel to its active metabolite are associated with an increased risk of cardiovascular events. Specifically, patients who are carriers of 1 ...

Impact of CYP2C19 genetic testing on provider prescribing patterns for antiplatelet therapy after acute coronary syndromes and percutaneous coronary intervention.

BACKGROUND Patients treated with clopidogrel who have ≥1 loss of function alleles for CYP2C19 have an increased risk for adverse cardiovascular events. In 2010, the US Food and Drug Administration issued a boxed warning cautioning against the use of clopidogrel in such patients. We sought to assess the impact of CYP2C19 genetic testing on prescribing patterns for antiplatelet therapy among pati...

Genetic Polymorphisms of CYP2C19 and Resistance to Clopidogrel Therapy among Iranian Patients Suffering from Ischemic Heart Disease

Background: Clopidogrel is a standout amongst the most ordinarily recommended medications to avoid ischemic occasions taking after coronary disorder or stant position. However, impaired responses the therapy as well as resistance to the therapy have also been reported. Genetic variants play an important role in clopidogrel biotransformation of its active metabolite that may subsequently influen...

The clopidogrel-statin interaction.

activity in influencing the antiplatelet effect of clopidogrel came from the work of Lau et al.4,5 Over a decade ago, those authors demonstrated that atorvastatin co-administration attenuated the antiplatelet effect of clopidogrel and the pharmacologic modulation of CYP 3A4 activity directly influenced the pharmacodynamic effect of clopidogrel. That ground breaking study opened the “Pandora’s b...

Frequencies of two CYP2C19 defective alleles (CYP2C19*2, and *3) among Iranian population in Mazandaran Province

Background: Cytochrome P450 2C19 (CYP2C19) is a polymorphically expressed enzyme that shows marked interindividual and interethnic variation. CYP2C19*2 and CYP2C19*3 are the most frequently identified defective alleles in Orientals and Caucasian poor metabolizers (PM). The aim of this study was to investigate the frequencies of CYP2C19*1, CYP2C19*2 and CYP2C19*3 alleles and CYP2C19 genotypes in...
